Brain Cysts from Tapeworm Larvae Behind Woman's Hallucinations

A 52-year-old woman in the Philippines experienced headaches and visual hallucinations for four months before scans revealed cysts scattered throughout her brain, each containing a tapeworm larva. Doctors diagnosed neurocysticercosis, an infection caused by ingesting microscopic eggs of the pork tapeworm Taenia solium from contaminated food or water. The case report, published in the New England Journal of Medicine, highlights how parasitic infections can present with neurological symptoms.
The scolex visible on brain imaging is a diagnostic hallmark of neurocysticercosis, appearing as a bright nodule within each cyst. The woman's language difficulties—fluent but nonsensical speech, impaired comprehension, and inability to follow commands—pointed to brain regions governing communication, distinguishing her from a primary psychiatric case. Her hallucinations were suspected to stem from seizure activity rather than psychosis.
Neurocysticercosis arises when Taenia solium eggs are ingested through fecal-oral contamination, often where sanitation is poor—the woman had spent time in evacuation centers the prior year. While treatable, the infection carries serious risks: dying larvae can provoke dangerous brain swelling, and cysts may obstruct cerebrospinal fluid flow. Similar cases elsewhere, including a Peruvian teenager with extensive brain involvement, show hallucinations and delusions can accompany this disease.
This case underscores how parasitic diseases, often associated with poverty and inadequate sanitation, can masquerade as neurological or psychiatric conditions. Misdiagnosis could delay treatment and prove fatal, so clinicians worldwide may need to consider neurocysticercosis when patients present with hallucinations and language deficits, particularly if they have traveled to endemic regions. Improved sanitation and food-handling practices remain crucial preventive measures, though the condition's global reach means awareness matters beyond endemic areas.
