FDA Flags Danger of Off-Label Kybella Injections
The FDA has updated its safety guidance for deoxycholic acid injection, warning that using it outside the approved submental area may lead to serious neuromuscular and vision problems. A review of adverse event reports found 129 cases linked to off-label use, including facial nerve injury and blurred vision. The agency also noted persistent injection-site nodules in over a hundred cases, sometimes requiring medical intervention.
The FDA's review of adverse event reports spans cases reported since Kybella's 2015 approval, with 129 adverse events tied to off-label use. Among the most concerning were vision disturbances linked to periorbital injections and facial nerve damage manifesting as muscle weakness or numbness. The agency also documented 117 cases of persistent injection-site nodules, some lasting an average of 143 days before resolution.
This warning builds on earlier FDA communications. In 2023, the agency cautioned against unapproved fat-dissolving products sold online, which had been linked to painful scarring and serious infections. At approval, the FDA had already flagged potential nerve injury in the jaw area, though the approved indication remained limited to submental fat.
This warning could affect patients seeking cosmetic fat reduction outside approved areas, particularly those considering off-label injections for jowls, cheeks, or other facial regions.
