Medicare Proposes Aligning Lab Test Payments with Commercial Rates, Cutting Rates
CMS has proposed 2027 rates for clinical laboratory services that would reduce Medicare payments by an estimated $1 billion annually. The agency says Medicare currently pays 16% more than commercial insurers for lab tests, and federal law caps annual cuts at 15%. Hospital labs, which perform up to 55% of tests, could be significantly affected.
The proposed rate changes stem from CMS's finding that Medicare lab payments exceed commercial rates by 16 percent. Because federal rules limit annual reductions to 15 percent, some codes will require several years of incremental cuts before reaching the target alignment. CMS expects savings of roughly $1 billion per year once fully implemented.
More than 1,000 laboratory codes face potential adjustment under the 2027 fee schedule. Hospital laboratories, which perform between 40 and 55 percent of all lab tests, would bear significant exposure to the reduced payments. CMS will publish final 2027 rates in November.
The proposed cuts could strain hospital laboratory budgets, particularly for facilities that rely heavily on Medicare reimbursement. Smaller hospitals and those serving older populations may face the greatest pressure, potentially affecting staffing, equipment upgrades, or test availability. Patients could see changes in where tests are performed or how labs operate, though Medicare beneficiaries themselves would likely see no direct change in coverage. The multi-year phase-in may give hospitals time to adapt, but cumulative reductions could reshape laboratory economics over time.
