Alignment Healthcare Says Six Medicare Advantage Contracts Earn Top CMS Ratings

Alignment Healthcare said six of its seven rate-eligible Medicare Advantage contracts received at least four stars in CMS’s 2027 ratings, including three contracts at 4.5 stars. The contracts cover Arizona, California, Nevada, North Carolina, and Texas, while its California H3815 HMO contract received 3.5 stars. Alignment disputed that lower rating and noted broader industry concerns about the Star Ratings methodology.
Alignment Healthcare reported that six of seven Medicare Advantage contracts eligible for CMS’s 2027 Star Ratings scored 4 stars or higher. Three of those contracts reached 4.5 stars, covering members across Arizona, California, Nevada, North Carolina, and Texas. Its California H3815 HMO contract received 3.5 stars.
The company challenged that result, saying it does not match the contract’s history on clinical quality, outcomes, and member experience. Alignment also pointed to wider industry questions about CMS’s rating methods, said it intends to pursue administrative remedies or litigation, and aims to return the contract to at least 4 stars. CMS published the 2027 ratings on Oct. 8, 2026, based partly on member feedback, service metrics, and clinical performance.
The ratings may shape how Medicare Advantage beneficiaries compare plans, especially in states covered by these contracts. Higher scores could reassure current and prospective members, while the disputed California rating may prompt some to seek more information or consider alternatives. The company’s challenge could also draw attention to how CMS evaluates plan quality, potentially affecting trust in ratings and plan accountability.
