Travere Highlights Biomarker Findings for FILSPARI in IgA Nephropathy

Travere Therapeutics presented new biomarker data from the Phase 3 PROTECT study at ASN Kidney Week 2026, indicating that FILSPARI may affect the underlying biology of IgA nephropathy. The company said FILSPARI produced greater reductions in biomarkers tied to kidney inflammation and injury than maximum labeled-dose irbesartan. The findings add to earlier proteinuria and kidney-function results and support the drug as a foundational treatment for IgAN.
At ASN Kidney Week 2026, Travere Therapeutics reported a biomarker analysis from PROTECT, a randomized, double-blind, active-controlled trial that enrolled 404 adults with IgA nephropathy; 295 joined the biomarker analysis. Through 110 weeks, FILSPARI (sparsentan) produced rapid, sustained declines in four urinary markers, with larger changes than the highest labeled irbesartan dose.
At week 110, the sparsentan arm showed greater reductions in sCD163, BAFF, C5b-9, and IL-6, markers tied to macrophage activity, B-cell signaling, complement activation, and inflammation. These data follow earlier PROTECT findings on proteinuria and kidney function and real-world evidence, according to the company.
If confirmed, these biomarker signals could give nephrologists another way to discuss disease-modifying treatment with IgA nephropathy patients, potentially affecting treatment decisions and monitoring. Patients and caregivers may face complex choices about a non-immunosuppressive option, while health systems could weigh costs and access. Because the data are exploratory biomarker findings, broader societal benefit would depend on whether they translate into durable clinical outcomes and real-world uptake.
