Compass Therapeutics Reports Tovecimig Trial Data in Biliary Tract Cancer

Compass Therapeutics published and presented Phase 2/3 COMPANION-002 results for tovecimig in advanced biliary tract cancer that had already been treated. The 168-patient study met its objective response and progression-free survival endpoints, with response rates of 18.0% versus 5.3% and median progression-free survival of 4.7 versus 2.6 months. Overall survival did not differ significantly, and grade 3 or higher treatment-emergent adverse events were more frequent in the tovecimig arm.
COMPANION-002 enrolled 168 people with advanced biliary tract cancer whose disease had progressed after earlier therapy. Investigators compared tovecimig added to paclitaxel against paclitaxel alone.
The combination produced tumor responses in 18.0% versus 5.3% and extended median progression-free survival to 4.7 from 2.6 months. Overall survival was not significantly different. Severe treatment-emergent side effects were more common with the combination (93.5% vs 64.2%), and fatal events occurred in 5.6% versus 1.9%.
For patients with limited options after prior treatment, the results may offer a new discussion point about response and progression-free survival, though the lack of an overall survival difference and higher severe toxicity could temper enthusiasm. Clinicians and caregivers may weigh benefits against added adverse effects; payers and developers could consider whether the regimen merits further study or access decisions. The broader public is affected mainly through how oncology evidence is communicated and how treatment choices are framed.
