Vitamin D deficiency linked to worse post-surgical pain and opioid use

A study of 184 women undergoing mastectomy found that those with vitamin D deficiency were three times more likely to experience moderate to severe pain in the first 24 hours after surgery. They also used substantially more of the opioid tramadol and had more postoperative nausea. The findings suggest that supplementation before surgery might help reduce pain and opioid needs.
The research followed 184 mastectomy patients at an Egyptian hospital over roughly eight months, splitting them evenly by vitamin D status. Those with levels below 30 nmol/L reported significantly more pain during their first day of recovery and required notably higher doses of tramadol through a patient-controlled delivery system. They also experienced more nausea after the operation.
Vitamin D's connection to pain processing may stem from its influence on inflammatory pathways and immune function, both of which shape how the body responds to surgical trauma. Because deficiency is common among breast cancer patients, the authors suggest that correcting low levels before surgery could potentially reduce both discomfort and reliance on opioid medications.
This study could influence pre-surgical care protocols, particularly for breast cancer patients, who already face elevated rates of vitamin D deficiency. If confirmed by larger trials, routine screening and supplementation before operations might reduce opioid consumption, easing the burden of postoperative nausea and potentially lowering risks of opioid dependence. Hospitals and surgical teams may need to weigh the costs of testing and supplementation against potential savings in pain management and recovery time.
